White Coat Hypertension in elderly with Isolated Systolic Hypertension From the population-based 11-country International Database on Ambulatory Blood Pressure Monitoring in Relation to Cardiovascular Outcomes database (IDACO), Franklin and co-workers studied the prognosis of white coat hypertension in subjects with isolated systolic blood pressures (ISH). Subjects with untreated ISH and white coat hypertension (WCH) had a similar low risk for cardiovascular events as normotensive untreated individuals. Subjects with antihypertensive treatment and an ambulatory blood pressure normalized to normotension had a higher risk for cardiovascular events than untreated normotensives. This was not unexpected. However, subjects with WCH and treated to normotension had a similar higher risk for cardiovascular events as those with sustained hypertension treated to normotension. Therefore, the authors suggest that the term white coat hypertension should be used with caution in patients treated for hypertension. Franklin SS, Thijs L, Hansen TW et al.
